Hour Self-Study Haz Mat Refresher
This year’s Haz Mat Refresher will be accomplished by completing:
8
hour Self-Study
Comprised of Drill A, Drill B, Drill C, Drill D and Drill E
in the August In-Service Packet available at all
CZU/San Mateo & Santa Cruz
County Fire Stations.
Completing all components will meet the mandatory 8 hour
annual
requirement.
This must be completed by: Sunday
October 28th, 2007,
in order to receive your certificate of completion.
Certificates will be issued no sooner than November 1st, 2007
after all Self-Study Verification Forms have been received in the
Training office.
SELF-STUDY REQUIREMENTS:
Once you have completed Drill A, B, C, D, & E have your supervisor sign the Self-Study Verification form and then submit:

Be Advised:
Keep copies
of your verification sheet in your station training folder.
You do not need to submit your completed tests.
Only the Self-Study Verification Form is required in the Training
Office.
